Obtaining your own MOS organizational license plate is easy! Complete Motor Vehicle Administration (MVA) Form VR-124. The form asks for typical owner and vehicle information, along with owner signatures. However, the form cannot be processed unless it is signed by the Mailing List keeper, Helen Horrocks. Her contact information is listed on page 2 of the Maryland Yellowthroat. You must send the form to her for certification before you can obtain your plates. Helen will return the signed form to you. Mail it to MVA or visit the Glen Burnie full service center to obtain your plates. Don't forget to enclose the $15.00 fee!
